
Medical Record Requests
In accordance with applicable state and federal privacy laws, we can release copies of medical records only to authorized individuals or entities. Records may be released to:
• You, the patient
• A parent or legal guardian (for minor patients, where applicable)
• A person holding a valid healthcare power of attorney or other written authorization
• Another healthcare provider involved in your care
• Insurance companies or payers, when required for payment or prior authorization
• Attorneys or legal representatives, when accompanied by proper authorization or a valid court order
• Government agencies or regulators, when required by law
If you are requesting records on behalf of someone else, you may be asked to provide identification and documentation (such as a photo ID, legal guardianship papers, or power of attorney).

Timeline & Fees
Processing of medical record requests typically takes 7–10 business days from the date we receive your completed request.
